Seventy-four million is not a patient count
On October 26, 2026, Celltrion said Omlyclo (omalizumab) and Avtozma (tocilizumab) had secured exclusive supply contracts with a large US healthcare organization. The group operates hospitals, insurance plans and pharmacy benefit management functions.
Omlyclo also entered a large retailer's pharmacy network, employee health plan and prescription discount program. The most striking number was 74 million. It represents members of that discount program overall, not 74 million people who received Omlyclo.
The important development is entry into channels where a medicine may be covered, supplied and prescribed. A large addressable population should not be confused with realized demand or revenue.
FDA approval and preferred formulary placement solve different problems
Omlyclo is an omalizumab biosimilar of Xolair; Avtozma is a tocilizumab biosimilar of Actemra. Biosimilars must be highly similar to their reference biologics, with no clinically meaningful differences in safety or effectiveness.
Even after US FDA approval, a product is not automatically covered on identical terms by every insurance plan. Payer and PBM formularies define coverage rules and preferred medicines, while hospitals and pharmacies make separate purchasing and dispensing decisions.
Celltrion launched Omlyclo in the United States on October 5 and announced preferred placement on a major PBM commercial formulary. Omlyclo also holds interchangeable status, which under applicable state law may allow pharmacy substitution. That status is not a universal mandate for insurers to prefer the product.
A healthcare group combines payer access with hospital supply
Celltrion's US unit signed a contract covering Omlyclo prefilled syringes and all Avtozma IV and subcutaneous formulations with a major healthcare group. Within that group's covered arrangements, both products were selected over the originators as exclusive supply products and preferred formulary medicines.
The company says product supply and reimbursement coverage began in October. This is more than an additional wholesale relationship: the payer pathway and supply to hospitals within the same group are connected, reducing some of the commercial friction between a covered medicine and the ability to obtain it.
However, exclusivity is confined to the specific agreement and product scope. It does not mean Celltrion replaced the originators across the whole US market, nor that physicians must prescribe the products. Treatment decisions, plan rules, clinical need and real procurement still determine volume.
A 350,000-member employer plan is different from a 74-million-member discount network
Under another agreement, a major US retailer began selling Omlyclo through in-store pharmacies. Omlyclo also achieved exclusive preferred positioning for applicable prescriptions in a health plan covering roughly 350,000 employees and dependents, with exceptions where certain vial presentations are needed.
The separate 74-million-member figure refers to the retailer's prescription discount program. Members, including eligible uninsured shoppers and dependents, may purchase Omlyclo at discounted prices through participating pharmacies. Discount-card eligibility does not itself constitute insurance reimbursement.
Neither figure measures patients with indications for omalizumab. They cannot be added together or converted into revenue, and no actual number of prescriptions was disclosed. The evidence shows distinct insured and non-insured commercial access routes.
Previous products can create relationships used for the next launch
Celltrion says its prior supply relationships helped secure the healthcare group agreement. The company had already supplied Stoboclo and Osenvelt (denosumab) to major hospitals within the same healthcare network.
This matters strategically because later products do not always start from zero. Sales operations, contracting experience and delivery relationships established through one product may support negotiations for another.
Still, a buyer's prior familiarity cannot guarantee future adoption. Formularies, clinical indications, net prices and competition must be evaluated separately for Omlyclo, Avtozma and any later products. The repeatability of these relationships is a hypothesis to test across successive launches.
European tenders and US PBMs are different gateways
Celltrion says Omlyclo has reached 26 European markets and exceeded 90% share in Spain as of July 2026. It also cited national tender wins in Denmark. These figures are company-reported outcomes for particular periods and markets, not a global share.
In the US, access also involves PBM formularies, employer-sponsored coverage, hospital networks and retail pharmacy arrangements. The entities deciding payment and purchasing differ between healthcare systems.
Global commercialization therefore involves more than transporting a licensed medicine from one country to another. Sales teams must understand who can list, reimburse, purchase and dispense a product in each market and what evidence is needed for those decisions.
The Q3 earnings cannot be attributed to October's US contracts
On October 27, Celltrion reported Q3 2026 revenue of KRW 1.5755 trillion and operating profit of KRW 525.3 billion. Newer products generated KRW 905.8 billion and 67% of reported biologics sales. Omlyclo generated KRW 320.5 billion in cumulative sales through Q3, supported mainly by European performance.
But the quarter ended in September. The US Omlyclo launch took place on October 5, and Celltrion announced the latest US channel agreements on October 26. October contracting cannot have caused the reported July–September earnings.
The observed sequence is a growing portfolio followed by new commercial access in the US. Whether this translates into higher prescriptions, net sales and margins will require future reporting.
Coverage, prescriptions and profit must be verified separately
The counterparties' names, contractual prices, expected prescription volumes and net sales for these arrangements were not disclosed. Additional major PBM formulary negotiations were described as near completion, not signed as of October 26.
The next useful measures are actual prescription volume by channel, continuing hospital orders, pharmacy net sales, discounts and contractual costs, and confirmation that additional PBM access is executed. Exclusive supply is not equivalent to guaranteed profits.
The broader business question is whether a distributor and payer network built through existing products can help multiple follow-on medicines secure meaningful repeatable access. Regulatory authorization is one achievement; a reliable pathway into clinical care is another.
BANSEOG VIEW
BANSEOG VIEW | Being allowed to sell is different from being positioned for prescribing
Celltrion's 74 million figure is discount-program membership, not treated patients. The stronger signal is access across a healthcare group's payer and hospital network, and separately a retailer's employee-plan and pharmacy discount channels.
The earlier Stoboclo and Osenvelt relationship offers a concrete example of how commercial partnerships may be reused for later products. The economics and scope of each formulary and supply deal still need independent evaluation.
Contracts signed in October should not be credited for Q3 earnings. Real prescription volume, net sales, payer coverage and contract profitability will determine whether the expanded network produces lasting value.
